I give permission for my child(ren) to participate in this youth event with Am Shalom. While I understand that every precaution will be taken with his/her care and transportation, I will assume any liability for any injury incurred. If I cannot be reached in the event of an emergency, I give officials permission to render any necessary emergency care.
